How to set two conditions on one boundary (1-D PDE)

Hi,

I need some help on setting a conditional boundry condition on a single point in 1-D problem. For example, for a single point in my model, i need to set the boundary condition for it. When the result h (always >0) of the this point < 5, the boundary condition for 0< h< 5 is A, while when the value of this point >= 5, then the boundary condition for 0< h< 5 is A, while for h>=5 is B.

Anyone knows how to solve this problem? Thanks in advance.
