Concentric Pipes flow

I am trying to simulate fluid flow through two concentric pipes. My model runs properly for low velocities (0.0005 M/S) but not higher once (0.5m/s). Attached are some pictures of the model. Please assist me.

Have you tried smoothed increase of inlet velocity?
I mean solve it for 0.0005 m/s, use the solution as an initial condition for let's say 0.001 m/s and so forth.
You may also use a parametric sweep study.
