Tetrahedral mesh and wave optics

I've made some models of an optical fiber with Comsol using a swept mesh from a triangular input port.
This works correctly and I get what I expect.
But, when I use a tetrahedral mesh the mode propagating in the fibre disappears and I get some output which looks like a mess. Is there anything I can do that will allow me to use a tetrahedral mesh, but still preserve my model?
I want to start adding asymmetric objects to my model which will not allow me to use a swept mesh everywhere.
Any advice is appreciated.
